This could be related to bug 0000568.

I added another dataset: hexContour.vtk. This dataset shows bug 0000568, but after using vtkDataSetTriangleFilter the display is fine. So to reproduce the bug use the VTK dataset : hexContour2.vtk

assigning this one back to Will because I don't have time to look at it

This filter relies heavily on cells being able to triangulate themselves, which is where I suspect the problem is.
